Covid-19 Preparedness Plan
Peak Physique is committed to providing a safe and healthy workplace for all our employees and customers. To ensure that we have developed the following COVID-19 Preparedness Plan in response to the COVID-19 pandemic. Employees are all responsible for implementing this plan. Our goal is to mitigate the potential for transmission of COVID-19 in our workplaces and communities, and that requires full cooperation among our employees, management and customers. Only through this cooperative effort can we establish and maintain the safety and health of our workplaces.

Employees are responsible for implementing and complying with all aspects of this COVID-19 Preparedness Plan. Peak Physique Employees have our full support in enforcing the provisions of this policy.

Our employees are our most important assets. We are serious about safety and health and keeping our employees working at Peak Physique. Employee involvement is essential in developing and implementing a successful COVID-19 Preparedness Plan. We have involved our employees in this process by working with our leadership team to discuss ideas, potential issues, and possible solutions; also by keeping open lines of communication to address future concerns that may arise. Our COVID-19 Preparedness Plan follows Centers for Disease Control and Prevention (CDC) and Minnesota Department of Health (MDH) guidelines, federal OSHA standards related to COVID-19 and Executive Order 20-48, and addresses:
• hygiene and respiratory etiquette;
• engineering and administrative controls for social distancing;
• customer controls and protections;
• housekeeping, including cleaning, disinfecting and decontamination;
• prompt identification and isolation of sick persons;
• communications and training that will be provided to employees; and
• management and supervision necessary to ensure effective implementation of the plan.

Screening and policies for employees exhibiting signs and symptoms of COVID-19
Employees have been informed of and encouraged to self-monitor for signs and symptoms of COVID-19. The following policies and procedures are being implemented to assess workers’ health status prior to entering the workplace and for employees to report when they are sick or experiencing symptoms. When each employee comes to work, upon entry they will go through a screening process. Each employee will have a personal file to log this information. It will include date, temperature, and questionnaire to be filled out as soon as they enter the building.

Every time an employee enters into Peak Physique they must be able to answer these questions.
1.) Have you had a new or worsening cough?
2.) Have you had a fever above 100.4?
3.) Have you had shortness of breath?
4.) Have you been within 6ft with anyone with these symptoms or anyone who has been diagnosed with Covid-19 in the past 14 days?

If the employee responds “Yes” to any of the screening questions, they will be asked to return home and wait to return to the facility until all symptoms, including fever have been resolved for atleast 72 hours without medication, or atleast 14 days after contact with a person with a cough, fever, or diagnosed with Covid-19.

They will notify their direct manager so accommodations can be made. Employees are also encouraged to monitor their health and any potential symptoms and notify their direct manager as soon as possible so accommodations can be made in advance. If an employee is tested for Covid-19 and has a positive test they will be asked to take two weeks off from physically coming to work at Peak Physique, or until otherwise notified by a medical doctor that they may return to work. In these scenarios remote working options will be discussed with their direct manager. Peak Physique will do its best to provide remote working solutions depending on but not limited to; current needs of the business, overall health and work ability of the employee, and current work required by the business.

Peak Physique has implemented leave policies that promote workers staying at home when they are sick, when household members are sick, or when required by a health care provider to isolate or quarantine themselves or a member of their household. Full time employees may take a two week leave in the event they need to self-quarantine; As directed by a Medical Doctor. If a Full time employee is required to self-quarantine, remote work will be discussed based upon the employee’s health and ability to perform work. Full time employees will receive one week of sick leave pay (this is in addition to the employees current PTO that is available. Part time employees may take a two week leave in the event they need to self-quarantine; as directed by a medical doctor. Part time employees will go un-paid during this leave unless there are remote work hours available (to be discussed with direct manager).

Peak Physique has also implemented a policy for informing employees if they have been exposed to another employee with COVID-19. Employees will be notified to monitor for symptoms and asked to self-quarantine if they show signs or symptoms of illness, have had a temperature above 100.4 degrees, have tested positive themselves for Covid-19, or have been directed by a Medical Doctor to self-quarantine. If employees did not come within 6ft of distance of the employee diagnosed with COVID-19 in the past 14 days, they can resume working.

In addition, a policy has been implemented to protect the privacy of workers’ health status and health information. In the event an employee test positive for Covid-19 no names or specific information will be given in regard to the employee if they have not signed a medical release form with Peak Physique. If a medical release form was not signed by an employee who tested positive for COVID-19, a question will simply be asked to other employees if they have come within 6ft of any employees in the past 14 days in the workplace. If the response is “yes”, and was within 6ft of the employee diagnosed, they will be asked to self-quarantine for 14 days, unless they are able to obtain a negative COVID-19 test result, they may return to work.

Screening and policies for customers exhibiting signs and symptoms of COVID-19
Customers have been informed of and encouraged to self-monitor for signs and symptoms of COVID-19. When each customer comes to Peak Physique an indoor workout, upon entry they will go through a screening process. Each customer will receive a temperature check. If a customer has a temperature above 100.4 they will be asked to return home and wait to return to the facility until all symptoms, including fever have been resolved for atleast 72 hours without medication.

Every time a customer enters into Peak Physique they must be able to answer these questions.

8.) Have you been within 6ft of contact with anyone with these symptoms or anyone who has been diagnosed with Covid-19 in the past 14 days?

If the customer responds “Yes” to any of the screening questions, they will be asked to return home and wait to return to the facility until all symptoms, including fever have been resolved for atleast 72 hours without medication, or atleast 14 days after contact with a person with a cough, fever, or diagnosed with Covid-19.

If an Employee at Peak Physique notices a customer is clearly exhibiting symptoms of illness, they would confirm if these symptoms are a new or worsening symptom. If answered “yes”, the employee may ask the customer to leave immediately and not return until at least 72 hours after symptoms have resolved without medication.
Customers who may be of higher risk to severe Covid-19 complications (persons over age 60 or with underlying medical conditions) are strongly encouraged to stay home and continue Peak Online.

Handwashing
Basic infection prevention measures are being implemented at our workplaces at all times. Employees are instructed to wash their hands for at least 20 seconds with soap and water frequently throughout the day, but especially at the beginning and end of their shift, prior to any mealtimes and after using the toilet. Hand sanitizer will also be available to use in place of washing hands when applicable. Hand sanitizer will be stationed at the entry of each building and is to be used immediately upon entry. Hand sanitizer will also be readily available in multiple locations throughout each facility (offices, front desk, lobby, workout floor). Hand sanitizer will be regularly filled by front desk employees to ensure there is always plenty to be used.

Customers must use hand sanitizer upon entry to facility, and is recommended to use as they leave the facility.

Respiratory etiquette: Cover your cough or sneeze
Employees and customers are being instructed to cover their mouth and nose with their sleeve or a tissue when coughing or sneezing and to avoid touching their face, in particular their mouth, nose and eyes, with their hands. They should dispose of tissues in the trash and wash or sanitize their hands immediately afterward. Respiratory etiquette will be demonstrated on posters (placed throughout each facility) and supported by making tissues and trash receptacles available to all Employees and Customers. Wearing of masks is highly welcomed by employees, but not encouraged when physically exerting their self (i.e. demonstrating exercises). Employees can choose to bring their own mask or use masks that will be made available at Peak Physique.

Social Distancing
Social Distancing will be implemented in four phases. Whereas each phase will allow more flexibility in how we operate. Indicated as Phase ONE, Phase TWO, Phase THREE, and Phase FOUR. The duration of each phase will be an ongoing adjustment with an emphasis on the health and safety of all employees and customers.
Social distancing is being implemented in the workplace through the following engineering and administrative controls:

Phase ONE (outdoor):
Upon (OUTDOOR) re-open Peak Physique will implement “Work in Place” Programming to help ensure the health and safety of all employees and customers. Peak Physique will practice social distancing by marking designated areas for employees and customers. Social Distancing stickers will be placed on the lobby floor for indoor transactions and bathroom use. Stickers will be spaced 6 feet apart to create a visual of how far apart people should stand. Customers will only be allowed to enter the facility to use the bathroom or purchase retail products. Only two customers may enter the facility at a time, and must follow and use social distancing.

The parking lot will have designated marks for each customer to stay on during their workout. Employees will have designated walkways to ensure social distancing is met.
Outdoor workouts will be able to accommodate 9 customers, plus one coach for a total group of 10 people.

During Phase ONE customers will not share exercise equipment during their workout. They will utilize “Work in Place” Programming to help accomplish this.

Front Desk and or Coaches will be responsible for checking in all customer reservations. This will be done using Ipads of which only employees will touch. Session reservations will be enforced. Customers must reserve their workout sessions to ensure there is adequate room for all customers and employees to practice safe social distancing. If a client shows up and has not reserved and all spots are filled the client will be asked to leave. If there is room the customer will be allowed to stay.

To ensure social distancing during the purchase of retail items A.) Client takes item and then stands on social distancing sticker while front desk employee completes purchase or B.) Front Desk Employee takes item and places on front desk for customer then steps back to ensure a 6 foot distance is maintained while customer takes item.

To help ensure social distancing amongst employees and to help preserve work force. There will only be two employees (physically) working at each facility at one time (One Front Desk, One Coach) During the pre-planning phase leading up to re-open, there may be more than two employees working at each facility and social distancing must be maintained during all activities.

Workout sessions will run for 45 minutes to provide a 25 minute window to clean and disinfect in between sessions, as well to give time for one group of customers to leave before the next group of customers comes in. Employees and customers are prohibited from gathering in close groups (everyone must maintain social distancing). Employees and Customers are prohibited from gathering in confined areas, and from using other workers’ personal protective equipment, phones, computer equipment, desks, cubicles, workstations, offices or other personal work tools and equipment. Employees and customers must refrain from any personal touch such as hugs, high fives, shaking of hands.
Session Schedule will be condensed upon official re-open and will grow/change based upon demand of the customers, but also the health, safety, and availability of all employees. Customers must reference Zen Planner to see current schedule. Customers are asked to please attend 2/3 times per week during Phase ONE (outdoor) workouts to help ensure everyone has an opportunity to partake. As Peak physique evaluates demand and need we will work to accommodate everyone to the fullest extent possible, considering the given parameters.

Session schedule during Phase ONE (outdoor) workouts will be weather dependent. If the weather is not in favor of an outdoor workout a mass text message will be sent to all clients for that specific day as to which sessions will be canceled.

Phase TWO (moving indoor):
Upon re-open Peak Physique will implement “Work in Place” Programming to help ensure the health and safety of all employees and customers. Peak Physique will practice social distancing by marking designated areas for employees and customers. Social Distancing stickers will be placed on the lobby floor to help people navigate their way to the workout floor. Stickers will be spaced 6 feet apart to create a visual of how far apart people should stand. The workout floor will be taped off in 8 foot by 8 foot squares to ensure each customer has adequate space and can practice social distancing. Coaches will have a designated square to instruct from, and when navigating the workout floor must stay on the tape lines (lanes) to ensure social distancing is practiced.

During Phase ONE customers will not share exercise equipment during their workout. They will utilize “Work in Place” Programming to help accomplish this. Customers will workout in the center of their 8’ by 8’ square.

Front desk employees will remain behind the front desk during the check in process to ensure they practice social distancing. All check ins for workouts will be logged by the front desk employees. No Ipads will be available for check in, front desk staff will check customers in. Session reservations will be enforced. Clients must reserve their workouts sessions to ensure there is adequate room for all clients and employees to practice safe social distancing. If a client shows up and has not reserved and all spots are filled the client will be asked to leave. To ensure social distancing during the purchase of retail items A.) Client takes item and then stands on social distancing sticker while front desk employee completes purchase or B.) Front Desk Employee takes item and places on front desk for customer then steps back to ensure a 6 foot distance is maintained while customer takes item.

Upon Official Re-Open; To help ensure social distancing amongst employees and to help preserve work force. There will only be two employees (physically) working in each facility at one time (One Front Desk, One Coach) During the pre-planning phase (leading up to re-open) there may be more than two employees working in each facility and social distancing must be maintained during all activities.

Session size will be dictated by the number of 8 foot by 8 foot squares fit on the workout floor. This equates to roughly half capacity of what was administered Pre-Covid-19. Workout sessions will run for 45 minutes to provide a 25 minute window to clean and disinfect in between sessions, as well to give time for one group of customers to leave before the next group of customers comes in. Employees and customers are prohibited from gathering in close groups (everyone must maintain social distancing). Employees and Customers are prohibited from gathering in confined areas, and from using other workers’ personal protective equipment, phones, computer equipment, desks, cubicles, workstations, offices or other personal work tools and equipment. Employees and customers must refrain from any personal touch such as hugs, high fives, shaking of hands.

Phase THREE:
During Phase THREE we will stop using “work in place” and begin rotational styles of workouts again. As well begin incorporating more equipment into the workout.
During this phase as customers rotate through their workout they will start at one station and complete all sets. Then all equipment touched will be disinfected before moving onto the next station.

Phase FOUR:
During Phase FOUR Peak Physique will return to full functional capacity. Multiple rotations in workouts with still a very high focus on cleaning and disinfecting. “Personal Touch” such as High Fives will be allowed.

Social Distancing in the Lobby
During Phase ONE and Phase TWO it will be important that good flow and social distancing is practiced in the lobby. As mentioned, social distancing stickers will be on the floor to direct and give adequate space to all employees and customers. No guest will be allowed to wait in the lobby (unless purchasing retail items) to help ensure everyone’s safety and to help ensure capacity is met. Cubbies will also not be available in the lobby and it is highly encouraged that customers and employees only bring into the facility that which is necessary. Keep in Mind during Phase ONE (outdoor) customers will only be allowed to enter the facility to use the bathroom or purchase retail items. It is highly recommended to keep personal belonging, such as shoes, bags, purses, etc in vehicles.

Virtual Coaching
It is the current plan and intent to continue online virtual coaching. As Peak Physique navigates Re-opening there will be ongoing discussion on how to continue to deliver our Online/Virtual coaching experience for those who do not yet feel ready to return to Peak physically, and or those that have come to enjoy the “at home” experience and wish to continue their health and fitness journey online.

Housekeeping
Regular housekeeping practices are being implemented, including routine cleaning and disinfecting of work surfaces, equipment, and areas in the work environment, including restrooms, offices, lobby, and workout floor. Frequent cleaning and disinfecting will be conducted in high-touch areas, such as phones, keyboards, touch screens, controls, door handles, copy machines, credit card readers, etc. Employees must wear gloves when doing deep cleaning after session begin and during intermissions of sessions.

Front Desk Employees will clean the lobby, sanitizing all high touch surfaces, door handles, floor, and restrooms after each session begins. Front desk will also disinfect computer, and all high touch points at front desk at start and end of their shift.

Front Desk and Coaches will clean the workout floor after each session ends. Wiping all equipment with disinfectant wipes, spraying the floor (Turf/Red foam) with specific disinfectant spray, wiping of walls with disinfectant spray and micro fiber mop, Mopping of black rubber floor.

Coaches should disinfect the coaches station using disinfectant wipes at the start and end of each shift. Including mic, Ipad, Cubby, and any other surfaces or devices that are required to perform within the coaching role. Mic foam covers so be sprayed and store in a zip lock bag in coaches cubby when not being used.

Customers must disinfect all surfaces touched after use of bathroom. Disinfectant wipes and spray will be in the bathroom for easily cleaning of all surfaces. Showers need to be sprayed with the provided disinfectant spray provided in the bathrooms after use.

Please Note: Showers will not be available during Phase ONE (outdoor).

Water Station
No dixie cups will be available at the water machine. It is recommended to please bring a pre-filled water bottle. If it is necessary you may fill your water bottle at the water machine. Please do not physically touch the water station with your hands.

Please practice social distancing around the water station by providing a 6’ distance to anyone who may need to fill their water bottle.

Communications and training
This Preparedness Plan was communicated through this written document, A recorded Video, and Zoom/Phone conference calls as needed to all employees on Date: 6/2/2020 and necessary training will be provided as it pertains to each employee’s role. Additional communication and training will be ongoing through similar documents, videos, and Zoom/Phone conference calls as we navigate changes during this pandemic. Instructions will be communicated to customers about how business will be conducted to ensure social distancing between customer to customer interactions and employee to customer interactions. Managers are to monitor how effective the program has been implemented by observing the flow and social distancing of customers and employees. Management and employees are to work through this new program together and update the training as necessary. This COVID-19 Preparedness Plan has been certified by Peak Physique management and was posted throughout the workplace. It will be updated as necessary.

FAQ’s and Responses
How we are not violating the executive order? The stay at home order was lifted on 5/18/2020, and gatherings of 10 people have been permitted with social distancing. We have not physically re-opened our gym for customers to workout inside. We are hosting workouts outdoors with adequate social distancing and groups of 10 people or less, which is allowed in the current executive order by the governor.

Who gave us permission to host outdoor workouts? The mayor and chief of policy for both respective cities have confirmed it is ok for Peak Physique to host outdoor workouts in our parking lot. Groups must not exceed 10 people. (9 Clients and 1 Coach).

What should you do if policy, department of health, or any other authority comes to Peak Physique? You should address them politely and professionally. Provide them a copy of our preparedness plan. Explain that we have been authorized by the Mayor (Fridley - Scott Lund – Phone # 612-860-3235) (St. Cloud – Dave Klies – Phone # 320-255-7201). Tell them we are operating within the Governor’s latest executive order. Call Tim Moes (Owner – Phone # 320-309-6735). Comply with any requests the authorities have.

How many times can clients come per week? During the first re-opening phase we ask clients to only reserve and come 2/3 times per week, so we can help make room for everyone. At this time, we are working the best we can while also working with in the parameters given to us. Please help us support everyone to the best of our abilities as we are very limited in what we are allowed to do. We are working hard to create a win win situation for all clients and employees. By working together, we can successfully navigate this time period together!

Class reservations? Please professionally remind clients that reservations are mandatory just as they are in restaurants right now. We must do this to stay within the guidance given to us. We also can not go over groups of 10 people. We want to do the right thing to ensure we can stay open and continue to serve people. If we break those rules, we are subjected to losing the ability to run workouts, so please help us in doing this.

Can People stay and roll out/hang out afterwards? No, we need to utilize this time to disinfect all touched surfaces as well, make time for one group to leave before the next group arrives, so we stay within the 10 person limit.

Can people bring their own equipment? Yes, You will have access to Peak’s equipment each day you come to workout. We will use limited equipment to make sure people are not sharing equipment right now. If you feel more comfortable bringing your own equipment you are more than welcome. We will accommodate the workout the best we can with what you bring. Equipment could include Dumbells, Bands, Mats, etc.

New waivers must be signed? Yes, each client needs to sign the new Covid-19 waiver when they return to Peak.
